Caregiver Affidavits

Who needs a caregiver affidavit?

When you need someone to make important choices about your child when you cannot, consider a caregiver affidavit. This form does NOT grant legal guardianship, but does allow:

  • Chosen relatives or non-relatives living with your child lives to enroll them in school and grant medical decisions on your behalf OR
  • Allows non-relatives to enroll your child in school and grant school-related medical care only.
NOTICE: We recommend renewing the caregiver affidavits at the beginning of each school year. Once completed, please bring the form to the Student Services Department Enrollment, with valid identification, to 350 Brookside Ave. Beaumont CA. 92223.
