Voluntary Benefits (BACME, BTA & CSEA)
Beaumont Unified School District provides various voluntary benefits such as life insurance, identity theft protection and legal assistance plans to all benefit eligible employees. Enrollment instructions, plan costs, and plan summaries are available below.
METLIFE LIFE INSURANCE
You may elect additional life insurance for yourself and your eligible dependents. You may apply for spousal life coverage up to 100% of your amount of coverage, not to exceed $500,000 or 5 times your annual salary and must be in increments of $10,000. You may also apply for supplemental life coverage for your child(ren) to a maximum of $10,000. Employee must be covered in order for spouse and children to be covered.
A MetLife Statement of Health (SOH) form is required for the employee and spouse, if spouse coverage is also being applied for. A SOH form is not required for children as coverage is guaranteed. The completed SOH form should be returned to the Benefits Office to forward to MetLife for approval. If you do not complete the required Statement of Health Form (SOH), you and/or your dependents will not be approved or enrolled in Voluntary Life insurance coverage.
